COTA - Contractual

RESPONSIBILITIES:

  1. Implement treatment program for each child based on set goals and objectives. Develop strategies/techniques for home program as well as providing individualized treatment directly to each child.
  2. Work as a team member as part of the multi-disciplinary team, consulting and collaborating with members of other disciplines.
  3. Responsible for individual documentation as required for State Early Intervention Program and Insurance reimbursement purposes.
  4. Write progress notes for each child including behavioral objectives and observations for each session.
  5. Write summary reports for community people who may require this information.
  6. Attend scheduled staff meetings (IFSP, MDC, IEP, Collaborations).
  7. Review client progress and update goals with Occupational Therapist when necessary or upon parent requests.
  8. Perform other duties as assigned.

HOURS:
Vary

QUALIFICATIONS:

  • Associates degree in Occupational Therapy from an accredited program; knowledge of developmental disabilities
  • Successful completion of National Certificate for O.T.A.
  • Current State of Illinois License
  • Early Intervention credentialed or eligible for credentials. Early Intervention Evaluation Credentials preferred
  • Valid Illinois Driver's License

Speech and Language Pathologist

RESPONSIBILITIES:

  1. Administer necessary evaluations and written reports upon each child's entry into the program and reassessment after the initial evaluation when deemed appropriate.
  2. Develop motor, feeding, and sensory processing related goals and objectives for each child.
  3. Implement treatment program for each child based on above goals and objectives. Develop strategies/techniques for home program as well as providing individualized treatment directly to each child.

Direct Support Professional

RESPONSIBILITIES:

Direct Support Professionals provide assistance for individuals with developmental disabilities in any environment, which the individual chooses to participate. Assist in providing opportunities for informed personal choice by the individual in area of work, school recreation, living arrangements, neighborhoods and community inclusion. Will treat individuals with respect and dignity while preserving their confidentiality and privacy. May include providing personal hygiene supports in people's homes or at their place of employment.

HOURS:
Full and Part time positions available.
Days/evenings/overnights/weekends.

QUALIFICATIONS:

  • High School Diploma or equivalent
  • Required to pass 8th grade literacy exam
  • Good written and verbal communication skills
  • Valid Illinois Driver's License, in good standing

Janitor (part time)

RESPONSIBILITIES:

Cleaning and maintaining assigned facilities, location, and work sites within the standards agreed upon by agency and it's Janitorial Services' customers. Examples of required duties would be: Clean and maintain floors, properly collect and dispose of trash/litter, properly clean glass, properly clean, disinfect and sanitize required areas (restrooms, break areas, etc.),properly dust, maintain grounds according to customer contract, and assist in maintaining an accurate inventory of Janitorial Services' supplies and assets. Ability to work independently with a minimum of supervision. Perform other tasks, consistent with qualifications or regular duties, as needed and/or assigned.

QUALIFICATIONS:

  • High School Diploma or equivalent
  • Valid Illinois Driver's License, in good standing
  • Must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds
